  • Provide support and advocation during live visits to ensure clients goals accomplished within a provider visit

  • Follow-up on unmet health needs that have not improved
  • Translating care issues to ensure maximum time of visit is used for care needs
  • Providing post office visit game plan

Continued Consultation Tele-Health

$400 / month

  • Provides access for 1 month of follow-up and support on initial recommended tele-health care needs
  • Focused time on client questions and concerns
  • Minimum of 2 follow-up tele-health calls, up to a maximum of 4 calls

Continued Care Hourly Support
