$650 / month

  • Face to face visits at the senior’s residence
  • Provides access for 1 month of follow-up and support with onsite and/or tele-health visits
  • Focused time on client questions and concerns
  • Maximum of 2 follow-up onsite visits
  • Maximum of 2 tele-health calls

Type of calls:

  • Provider office: Primary or Specialty offices
  • Insurance carrier
  • Home health
  • Private duty
  • Pricing inquiring for Cash pay/self-pay options
  • Caregiver support
  • Provide support and advocation during live visits to ensure clients goals accomplished within a provider visit

Example of Needs:

  • Ordering needs
  • Follow-up on unmet health needs that have not improved
  • Translating care issues to ensure maximum time of visit is used for care needs
  • Providing post office visit game plan

Unlimited VIP Monthly Support

$850 / month

This does not include in-person face-to-face visits.

  • Provide access for 4 consecutive weeks, up to a max of 8 hours or 480 minutes per month
